Lifestyle Compass is designed to strengthen this personalised prevention and follow-through within the existing Health Check pathway.
Lifestyle Compass supports the existing NHS Health Check. It does not replace clinical assessment, QRISK, diagnosis, treatment decisions, the provider’s responsibility for abnormal findings or the authoritative GP record.
Assessment
Captures lifestyle strengths, priorities, barriers and readiness alongside relevant categorical risk information.
Personalised conversation
Helps the resident and practitioner identify which prevention opportunities are most relevant and agree one realistic goal.
Follow-through
Links the agreed goal to appropriate local support and records delivery and engagement where this is included in the commissioned pathway.
Initial feasibility work would examine acceptability, completion, consultation fit, goal-setting, signposting, follow-up and the quality of pathway documentation. It would not initially test clinical effectiveness.
